We currently have two Social Prescriber Link Workers, who work on a Tuesday & Thursday at the Practice. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support. If you want to speak with one of the Link Workers contact the surgery for an appointment.
